FDOH-Columbia COVID-19 Vaccination Registration Program

LAKE CITY, FL --  The Florida Department of Health in Columbia County has issued a statement asking county residents to register with the department for COVID-19 vaccinations if they wish to receive the vaccine. The statement reads as follows:

"The Columbia County Health Department is asking Columbia residents 18 and older that would like to have a COVID-19 vaccine to sign up now.  Starting Monday (3/15/21), all individuals over 60, as well as other groups identified by the Governor, are eligible for a COVID-19 vaccine.  In the future, each time Governor DeSantis opens new eligible groups, we will already have the information needed to send scheduling appointment information electronically (text/e-mail) and via phone. 

If you feel you signed up in our previous system and were not contacted, please call our COVID Hotline at 386-406-7006.

The scheduling site can be accessed in any of the following methods:
